Obtaining Copies of Medical Records

Patients have the right to obtain copies of their medical records maintained by the Health Information Management Department.

All requests for medical records must be submitted in writing and be signed by the patient or legal representative prior to the release of any medical records.

  • If the patient is a minor (under the age of 18), parental/guardian consent is required.
  • Legal representatives must provide Power of Attorney/proof of guardianship paperwork along with the written authorization form.
  • For deceased patients, a copy of the death certificate and proof of status as administrator of the estate must be provided.
  • A court order is required along with a State of Delaware Subpoena to release medical record information without patient/legal representative authorization.

All sections of this form must be completed to process your request including dates of service requested (approximate dates if exact dates of service are unknown) and the specific information to be released.

  • The form must be signed and dated by the patient or legal representative.
  • For verification purposes, include a copy of your government issued ID.
  • Include a phone number and/or email address so that we may contact you in case of any questions.
  • Please be aware that only records of visits within the last ten years are available per Rockford Center’s record retention policy.

The completed authorization and supporting documents can be sent via US mail to Rockford Center:

Every effort will be made to respond to your request within 7-10 business days; however, it may take up to 30 days to complete the request depending on the availability of the information.

Note: there may be a reasonable cost-based fee assessed for photocopying the medical record.  Copies of discharge paperwork are provided to the patient free of charge.  There is no fee charged to forward copies of medical information to your aftercare provider.
